Does OSCAR smear the cement over the bone surface?

As mentioned above the OSCAR system removes cement in layers. The final layer will reveal the bone. Once the cement has been removed any thin layer of cement that may remain will be normally be reamed away.

Will OSCAR damage bone?

No. Animal studies have demonstrated that when the tip of the probe is in contact with the endosteal bone surface for a period of 10 seconds, cell death to a depth of only 50 microns occurs. This contrasts with cell death to a depth of 500 microns, which occurs when conventional PMMA is applied to the endosteal surface.

Are the fumes dangerous/carcinogenic?

No more hazardous than the fumes created when the cement is mixed. The fumes have been fully tested; the dominant components are methylmethacrylate, benzine and styrene. All concentrations produced are well below OES/MEL values. (OES = occupational exposure standards, MEL = maximum exposure limits).

Will OSCAR force cement further into the interstices of the bone?

No, OSCAR removes cement a layer at a time. Any cement that remains in the interstices of the bone will normally be removed when it is reamed prior to a new stem being introduced.

How does it work?

OSCAR produces high frequency ultrasound; this liquefies PMMA bone cement to aid in its fast and efficient removal.

What happens if the probes get stuck in the cement?

When the tip is energised and light pressure applied the ultrasound that is produced during the tuning cycle will be enough to start to liquefy the cement surrounding the tip. If a full tuning cycle is not possible i.e. OSCAR can not find a suitable frequency to lock onto, this process should be repeated until enough room is created around the tip to allow the tuning cycle to be completed and full power is restored. At this stage the probe can be removed, cleaned and reinserted and the surgery continued as normal. It is very important that the handset is not bulled and too much manual force applied in attempt to remove the tip as this will hinder the tuning cycle and affects of the ultrasound liquefying the cement.

Will OSCAR damage soft tissues?

If the energised tip of OSCAR comes into contact with the patient's soft tissues during use the vibrating tip will cause rapid heating and therefore a friction burn. The tip of the probe should not be withdrawn from the bone canal under power. Also any area of tissue that will come into contact with either the horn or probe due to the angle of insertion should always be protected with a dry swab.
